Résumé

La résistance aux antibiotiques se considère aujourd’hui l’une des plus grandes menaces qui pèsent sur la santé mondiale. En fait, elle ne cesse de se progresser de manière alarmante, à la lumière de la surconsommation mondiale de ces médicaments, citant également le manque de développement de nouveaux agents antibiotiques par les laboratoires pharmaceutiques qui accentue la propagation de l’antibiorésistance. Dans ce contexte, nous avons mené une étude rétrospective concernant les examens cytobactériologiques des urines réalisés au sein de 5 laboratoires au nivau de la ville de Tétouan.
Le but de notre travail a été d’évaluer le taux de résistance de différentes souches bactériennes uropathogénes collectées à partir des laboratoires de la ville de Tétouan, afin de faire le point sur l’épidémiologie des infections urinaires au niveau local et sur les mesures à prendre pour freiner le phénomène grandissant de l’antibiorésistance.

Notre étude a montré que les femmes sont les plus touchées (75%). L’analyse de la sensibilité aux antibiotiques a montré une fréquence élevée de résistance aux différentes familles d’antibiotiques. Les entérobactéries étaient responsable de 78% des infections. Dans un total de 359 souches, E.coli dominait le profil épidemiologique (59%), suivie par Klebsiella pneumoniae (12%), Streptococcus pneumoniae (7%) et Proteus mirabilis (6%). Des taux élevés de résistance ont été observé à l’amoxicilline, l’amoxicilline + acide clavulanique et la doxycycline notamment chez les entérobactéries avec des taux de résistance compris entre 52% et 76%. Par contre les Streptococcus pneumoniae avaient des taux de résistance très faible allant de 4,1% à 8,8% à l’amoxicilline et l’amoxicilline+Ac.clavulanique. Une résistance importante aux céphalosporines de deuxième et troisième génération a été signalé, notamment au cefaclor, au cefixime et à la cefuroxime avec des taux compris entre 56% à 65,5%, à l’exception de la ceftriaxone, la ceftazidime qui présentent une activité relativement importante contre E. coli. Il faut noter que les Staphylococcus aureus ont présenté une résistance trés importante contre toutes les céphalosporines testées avec des taux allant de 86% à 100%. L’imipénème, l’amikacine, la colistine et la gentamicine étaient très actifs sur les souches étudiées qui ont montré de faibles taux de résistance compris entre 2,4% et 16,6%.

Abstract

Resistance to antimicrobials has been described as one of the major health problems all over the world. As a matter of fact, this growing challenge is fueled by the overuse of antibiotics, along with the lack of development of new antibiotic drugs by pharmaceutical companies which has significantly contributed to the spread of this phenomenan.

Against this background, we have conducted a study that concerns urinary tract infections in the city of Tetouan. The aim of this study was to evaluate resistance levels of certain strains of uropathogenic bacteria to provide an update on the local epedimiology of urinary tract infections in order to shed light on the measures to be taken to combat uropathogens resistance to antibiotics.

Our study revealed that women are most infected (75%). Analysing antibiotic susceptibility profile showed a high frequency of resistance to the main families of antibiotics. Enterobacteriaceae were the main cause of urinary tract infections (78%). In a total of 359 bacterial isolates, E.coli was the predominant pathogen (59%).The next three pathogens of importance were Klebsiella pneumoniae (12%), Streptococcus pnemoniae (7%) and Proteus mirabilis (6%). The activity of amoxicillin, amoxicillin+clavulanic acid and doxycyclin was greately reduced notably against enterobacteriaceae with resistance levels of between 52% and 76%.By contrast, amoxicillin and amoxicillin+clavulanic acid were effective against Streptococcus pnemoniae that showed low resistance levels of between 4,1% and 8,8%. A great resistance was signaled to cephalosporins of second and third genaration notably to cefaclor, cefixim and to cefuroxim with susceptibilities of between 56% and 65,5%, except for ceftriaxon and ceftazidim which showed relatively high activity against E.coli. Imipenem, amikacin, colistin and gentamicin were highly actif against the pathogens wich exhibited low resistance levels of between 2,4 % and 16,6%.
